Integrated Substance Abuse Care

A Coordinated Care Plan

Our Integrated Substance Abuse Care program brings together primary care, psychiatry, behavioral health, and other in-house services to support individuals with substance use concerns through coordinated, evidence-based care. Substance use is addressed alongside physical and behavioral health needs as part of routine, whole-person care — not as a separate or standalone condition.

By combining clinical assessment, therapeutic support, and skill-based programming, care teams work together to build personalized treatment plans that reflect each patient’s needs, goals, and preferences. All providers play a role in screening, responding, and supporting patients when substance use concerns are identified.

Care is delivered in a supportive, nonjudgmental environment and designed to meet patients where they are — in person, virtually, or through a hybrid model.

Integrated Care Services

Based on individual needs and preferences, care plans may include one or more of the following supports:

  • Assessment & care coordination
  • Individual behavioral health services for the whole family
  • Early recovery & relapse prevention group therapy
  • Individual substance abuse counseling
  • Medications for opioid use disorder (MOUD) options when appropriate

Meet The Team

Experienced Providers, Collaborative Care

Substance use concerns are addressed by providers across our organization as part of routine primary care and behavioral health services. Screening, assessment, and care planning are shared responsibilities across the care team.

Some providers offer specialized, in-house services that support integrated care plans, including group-based treatment and medication support when appropriate.

Recovery Support Groups

As part of our Integrated Substance Abuse Care program, we offer structured, clinician-led group services that focus on skill development, recovery support, and relapse prevention. These groups are designed to complement individual care and provide additional structure and peer support.

Wednesdays & Fridays | 9:00–10:00 AM

Thursdays | 1:00–2:00 PM

Individual Counseling

River Valley Family Care: Offered each Tuesday
Ripley Family Care: Offered each Wednesday, Thursday, and Friday.
