When attending your first appointment with Coplin Health Systems, you should bring the following items:
- Insurance Card
- Photo ID
- A list of your current medications
- Completed New Patient Forms
Coplin Health Systems is proud to offer Same Day Care options, including Urgent Care and Work In Care. Please visit our Same Day Care Options page, for specific Walk-In care hours for each location.
- Urgent Care services are offered at 7 locations, and include treatment for:
- Minor Illnesses: such as colds and flu
- Infections: such as UTIs, ear infections and more
- Minor Injuries: such as cuts, sprains, and burns
- Work In Care services are offered at 6 locations, and are exclusive to current patients only. Work In Care services are offered during regular business hours.
We are committed to your care, even outside of our regular office hours. Your well-being is our top-priority, and we’re dedicated to providing care whenever you need it.
If you require assistance after-hours, please call 304-531-1502. In the event of a medical emergency, please call 9-1-1 immediately. Your safety matters to us, and we want to make certain you receive the urgent care you need.
Yes – using our Patient Portal or the Healow App allows patients to manage their care online, including accessing your medical records at your convenience.
We provide a range of services to children, adolescents and the school community. Our school-based health clinics offer preventive, primary, and same day care, behavioral health, dental care and health education services. Referrals to other health care providers are available as needed.
